What to Expect (Job Responsibilities):
- Provide crisis intervention services in a home-based environment with flexible scheduling
- Conduct comprehensive assessments of family strengths, needs, and barriers to goal completion
- Use various teaching methods to help family members acquire and maintain necessary skills
- Collaborate with community resources and advocate for families while preparing them for service transitions
- Participate in program reporting and Continuous Quality Improvement processes
What is Required (Qualifications):
- Master's Degree with 3 years of relevant experience in community-based health or case management, or a Master's degree in Social Work or Mental Health Counseling
- Licensure as LMSW, LCSW, LMHC, LMFT, or LCAT
- Experience in the Homebuilder model of crisis intervention
- Valid NYS driver's license required

- Experience in a similar role within the mental health field
- Familiarity with crisis intervention techniques and behavior management strategies
- Knowledge of local community resources and support systems
