- Wide range of CPD activities related, which may include studies up to Masters level, related to the relevant specialist area
- Relevant accredited courses
- Relevant Masters Degree
- Clinical Supervisor accreditation
- Membership of RCOT/CSP
- Significant post-graduate experience in the relevant specialist area(s)
- In depth knowledge of research and policies relating to specialist area
- Demonstrates excellent clinical reasoning skills relating to working with complexity
- Well established knowledge of a broad range of assessment tools and of appropriate therapeutic interventions relevant to the Specialist area.

Job summary
Community Urgent Care Team is a 24/7 service; providing a first point of contact and comprehensive triage service, telephone and clinical assessment to patients and/or their carers/relatives resulting in a safe and appropriate clinical decision and plan of care.
The team is responsible for health, therapy and social care triage including supporting aversion of hospital admission or early hospital discharge through assessment and treatment of individuals in crisis, assessing complex and specialist needs in order to develop, implement and evaluate programmes of care. Supporting individuals to remain in community settings, in their own home, residential home or nursing home.

Senior Physiotherapist/Senior Specialist Occupational Therapist
Community Urgent Care Team
Westgate Park. Charlton Street, Grimsby
Hours: 37.5 Permanent
Rota Pattern: You will be required to work weekends and Bank Holidays
Salary Range 6 £35,527 - £42,663 per annum.
The postholder will provide a high standard of Occupational Therapy service undertaking all aspects of clinical duties as an autonomous practitioner.
You will manage a defined caseload of patients using evidence-based practice and models of practice to provide a patient centred assessment and to plan, implement and evaluate interventions, including discharge planning.
You will promote the importance of Occupational therapy within services including the wider MDT and external organisations .
You will also liaise with the wider MDT, supervise, educate and assess occupational therapy students, assistants and junior occupational therapists and provide specialised advice to other health care professionals in the specific clinical area.
Undertake evidence-based audit and research projects to further own and teams clinical practice within each rotation, making recommendations to Clinical Lead/Manager of service for changes to practice by the team. You may lead the implementation of specific changes to practice or contribute to development of service protocols.
