      Position Responsibilities:

      • Responsible for the overall Sepsis Program crossing many unit-based lines to follow all aspects of sepsis care. Ensures that all protocols and quality of care meet evidenced-based guidelines (Sepsis Bundle Elements).
      • Keeps abreast of regulation changes and required reporting, documentation, and protocols. Follows updated sepsis criteria released by the National Hospital Inpatient Quality Measures and promptly implements the changes.
      • Demonstrates understanding of quality management and performance improvement (PI) methodology and principles in daily work. Utilizes these principles to develop and maintain quality and PI and to educate staff and departments in sepsis-related patient safety concepts, processes, and tools.
      • Reviews daily admission log for patients admitted with key diagnoses and responds in person while on duty to Sepsis Best Practice Alert (S-BPA).
      • Reviews diagnostic laboratory and imaging results/reports for patients who trigger S-BPA for indications of infectious processes that could be the cause for development or progression of sepsis.
      • Responsible for follow-up on patients from off-hours sepsis alert S-BPA report the following work day and be available for clinical guidance while off duty.
      • Performs sepsis assessments and chart reviews of suspect patients promptly to determine eligibility for the sepsis program.
      • Serves as a resource person on the standards for "Early Management Bundle for Severe Sepsis/Septic Shock."
      • Works with the patient's attending physician, bedside nurse, team leader, local clinical leadership, and/or Rapid Response Team (RRT) to provide direction on the clinical care necessary to treat patients with suspected or actual sepsis, as per sepsis bundle elements and the required measure data reported to the National Hospital Inpatient Quality Measures "Sepsis Bundle Project."
      • Reviews and evaluates nursing recognition and quality of care of patients with early or developing sepsis, severe sepsis, or septic shock.
      • Provides feedback to clinical staff and collaborates with local leadership on methods to improve compliance with the sepsis program requirements.
      • Provides Sepsis and Sepsis Bundle education, guidance, and recommendations on improving care for patients with early or developing sepsis, severe sepsis, or septic shock, and the Sepsis Program to all staff. Provides other information and guidance as the situation warrants.
      • Collaborates with medical and nursing staff to ensure physician order sets have been implemented and utilized.
      • Collaborates with other healthcare team members through consistent daily rounding to assist in the planning, implementation, and evaluation of care delivery of septic patients.
      • Collaborates with Infectious Disease, Emergency Department, Medical, and Surgical Physicians regarding the treatment of septic patients.
      • Advises the clinical supervisor, Patient Safety Officer, Director of Infection Prevention and Control, and Hospital Epidemiologist of any issues with medical or nursing staff refusing to follow established medical guidelines.
      • Enters incident reports for care related issues and communicates with Risk Management. Refers cases and works with Medical Staff Quality on peer review of cases not meeting criteria.
      • Compiles and analyzes program-related data for internal review of performance and program effectiveness. Responsible for reviewing performance measures, making recommendations for improvement, and ensuring the data, analyses, and action plans are submitted to the Sepsis Program Committee and then up through the committee reporting structure as indicated.
      • Organizes and leads regular Sepsis Program Committee meetings with Physician Sepsis Champions.
      • Attends and/or contributes to the following committees regularly: Infection Prevention and Control, Patient Safety (PSC), Any PSC sub-committee as needed, Medical Staff Quality and Performance (MSQP), and upon request Medical Executive Committee (MEC) and Nursing Executive Committee (NEC).
      • Participates in ongoing education by attending conferences, webinars, journal reading, grand rounds, etc.

      Qualifications:

      • RN with recent ICU experience or ED experience. ACLS preferred.
      • BS in Nursing preferred
      • Experience in PI or Infection Control helpful, but not required.
