What to Expect (Job Responsibilities):
- Assist in developing and coordinating holistic care management activities for Medicaid LTSS members
- Evaluate member needs and recommend plans for optimal healthcare outcomes
- Coordinate services between members, families, and care provider teams
- Provide resource support for various member needs, including housing and employment
- Monitor care plans and provide recommendations based on member status and outcomes
What is Required (Qualifications):
- Bachelor's degree and 1 year of related experience, or equivalent experience
- For Illinois Plan: RN licensed in Illinois, or Bachelor's/Master’s Degree in a human services-related field, or LPN with one year of experience in conducting assessments for the elderly
- Knowledge of long-term care services and benefits
- Strong communication and interpersonal skills
- Ability to comply with state, federal, and third-party payer regulations
How to Stand Out (Preferred Qualifications):
- Experience working with Medicaid LTSS members or in a similar healthcare role
- Familiarity with community resources for long-term care
- Skills in developing and monitoring care/service plans
- Background in social work or counseling
- Bilingual abilities in relevant languages
