What to Expect (Job Responsibilities):
- Conduct telephonic outreach that meets/exceeds quality metrics and production targets
- Perform comprehensive assessments and document findings in compliance with department requirements
- Provide members with internal and external resources to improve Social Determinants of Health
- Collaborate with market Social Workers, members, PCPs, and specialists as needed
- Assist with provider outreach for notification of gaps in care and requesting medical records
What is Required (Qualifications):
- Bachelor’s in Social Work (BSW) from an accredited program
- Hold active, unrestricted Compact Licensed Social Worker (LBSW) in the state of work
- 2+ years of experience actively working as a social worker
- 2+ years of clinical background in a healthcare delivery setting
- Proficient in PC software skills such as Microsoft Word, Excel, PowerPoint, and Outlook
How to Stand Out (Preferred Qualifications):
- Graduate of an accredited Master's degree Social Work Program
- Current and unrestricted LBSW license in one or more specified states
- Active/unrestricted licensed Masters in Social Work (LMSW)
- Experience working with geriatric patient populations
- Bilingual preferred with preference given to Spanish and Vietnamese
