      Job summary

      Community Stroke Rehabilitation in County Durham are looking for a qualified Physioherapist with experience in stroke rehabilitation to join our successful team. The available post will be based at Chester le Street Hospital, covering the surrounding areas. The successful candidate will work alongside Occupational Therapsits, Speech and Language Therapists, and skilled associates to deliver innovative, best practice, evidence-based stroke rehabilitation in patients own homes and out-patient settings to optimise recovery.

      This is an excellent opportunity for an experienced physiotherapist to learn more about treating stroke patients in the community in a fantastic team. The post is available immediately. This is an exciting role in a challenging setting for someone who is able to organise their own busy caseload. Please call for an informal chat about the role.

      Main duties of the job

      Carry out intricate neuro assessments with patients within their own environments. Identify goals with patients, conduct reviews, create exercise programmes and monitor ongoing rehabilitation.

      Conduct supervisory duties with other team members and contribute to CPD

      Feed into weekly team MDT

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

      1 Specialist Clinical Practice:

      1.1 Maintain specialist clinical skills and provide expert advice to the area in relation to identified specialism

      1.2 To organise and prioritise own workload and to monitor those you clinically supervise within a defined area

      1.3 To independently screen, prioritise and plan interventions for routine and urgent referrals received within the specialist area

      1.4 To use sound clinical judgements to prioritise patients within a demanding and unpredictable environment ensuring effective use of resources. To assist staff under your supervision to prioritise their caseloads and advise them on appropriate interventions in response to urgent/and or complex cases on a daily basis

      1.5 To undertake holistic and comprehensive specialist assessments of patients including those with diverse or complex presentations/multi pathologies that change across the caseload. To utilise analytical and clinical reasoning skills and manual assessment techniques to provide accurate diagnosis of their condition, these skills may be advanced; interpret facts and select the appropriate treatment and implement rehabilitation programmes

This will involve using a range of verbal, written and demonstration techniques to overcome any barriers to understanding caused by physical disability (e.g., stroke) sensory impairment (e.g., deafness), cognitive impairment (e.g., confusion) or emotional factors

      2 Leader of Clinical Practice:

      2.1 To communicate effectively with patients and carers to maximise rehabilitation potential and to ensure understanding of their condition

      2.2 To liaise with, refer to and advise the multidisciplinary team and other professionals in regard to the management and discharge of patients in your care.

      To take delegated responsibility from more senior therapist in managing patients with particular conditions and be responsible for providing comprehensive physiotherapy assessment and treatment for patients with these conditions in a designated specialism

      2.3 To take delegated responsibility from more senior therapist in managing patients with particular conditions and be responsible for providing comprehensive therapy assessment and treatment for patients with these conditions in a designated specialism

      2.4 To be professionally and legally accountable for all aspects of your work, including management of patients in your care

      2.5 To communicate effectively and work collaboratively with medical, nursing and therapy colleagues to ensure delivery of a co-ordinated multi-disciplinary service
