- To participate in the assessing, planning, implementation and evaluation of individualised care for medical patients.
- To assist the MDT with providing clear and concise instructions to patients regarding treatment plans, medications, follow-up care, and self-management strategies, promoting patient empowerment and adherence to healthcare recommendations.
- To work collaboratively with all members of the MDT to develop and implement comprehensive care plans tailored to individual patient needs, ensuring continuity of care and effective communication across interdisciplinary teams.
- To recognise changes in patients conditions which require the intervention of others and refer on / escalate as appropriate.
- To clearly present the patients point of view to others, including promoting and protecting the interest and dignity of patients.
- Maintain accurate and thorough electronic patient records of patient assessments, interventions, responses to treatment, and discharge instructions.
- Ensure own practice is evidenced based and contribute to the development of nursing practice in own area.
- To work within the NMC code of professional practice, and within policies, procedures and guidelines of Calderdale & Huddersfield NHS Trust. To ensure that high standards of nursing care are given and maintained. To provide mentorship and preceptorship as required To support the Sisters in providing a suitable & effective learning and development environment for students and other staff members. To take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with reregistration requirements .

Job summary
This is an exciting opportunity to join the newly formed Medical Same Day Emergency Care (SDEC) Unit within the Calderdale Royal Hospital.
As a Registered Nurse (RN) working within the Medical SDEC Unit, you will play a pivotal role in delivering high-quality, patient-centered care to individuals requiring medical attention for acute conditions.
The Medical SDEC is a fast-paced working environment, where you will collaborate closely with a multidisciplinary team of professionals to assess, diagnose, treat, and manage patients. The successful candidates should be able to demonstrate, critical thinking, and effective communication whilst also adhering to established protocols and standards of care.

We employ more than 6,500 staffwho deliver compassionate care from our two main hospitals,Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, healthcentres and in patients' homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.
We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children's and young people's services; end of life care and outpatient and diagnostic imaging services.
We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.
