      Job Summary:
      The System Formulary and Medication Use Policy Manager is responsible for establishing/harmonizing and maintaining medication use policies required for regulatory compliance while reducing formulary variation and deviation across the health system. The System Formulary and Medication Use Policy Manager will collaborate closely with partner hospitals and their Pharmacy teams to inform medication use policy, formulary and clinical service harmonization and maintenance. The emphasis will center on the health system in optimizing clinical and regulatory performance.

      Education:
      Bachelor’s degree from an ACPE accredited School of Pharmacy required.
      Doctor of pharmacy degree preferred.
      Licensed Pharmacist in the state of Vermont or New York.

      Experience:
      PGY1 pharmacy and PGY2 residency training or at least three years of health system pharmacy
      experience required, preferably with experience in formulary management and crafting or maintaining medication use policy.
