      Position Summary:

      This individual will be an active member of the Targeted Case Management Unit (TCMU) within Philadelphia’s Behavioral Health and Intellectual disAbility Services (DBHIDS), Behavioral Health Division. A key dimension of this position will be the work performed with the Personal Care Boarding Home (PCBH) providers as it relates to current placements, potential placements, payments, linkages, and supports. This position will be involved in a variety of “special initiatives” that pertain directly/indirectly to TCM service development across the behavioral health system. As a member of the TCMU, this position will also provide targeted case management units located in various community mental health centers and specialized agencies with technical assistance and support pertaining to identified problems and issues that may need to be addressed on an individual, agency, or systemwide basis.

      Duties and Responsibilities:

      This individual will:

      • Function as primary point within the unit for fielding calls related to PCBHs, processing initial authorizations, monthly purchase requisitions for payment, assessing and screening of potential placements/referrals and on-site monitoring of select homes.
      • Work directly with PCBH proprietors regarding placements, expectations upon placement, discharges, etc.
      • Support work of PCBH facilities by acting as liaison between proprietors, TCM, and case management entities to assure that participant needs are being pursued/met
      • Understand current federal, state and county regulations regarding PCBHs as well as Case Management
      • Provide back up functions to the Special Initiatives Coordinator, particularly as it relates to PCBH requests (i.e., closures, unlicensed home identification, participant relocations, etc)
      • Participate in data entry functions as related to PCBH operations
      • Participate in the preparation and submission of reports on program activities and achievements and on progress toward overall goals as it relates to both TCM and Special Initiative functions
      • Advise TCM agencies on agency specific activities related to the Targeted Case Management rules and regulations as determined by DHS and adopted by the DBHIDS.
      • Provide TCMs, Supervisors, and Directors within the provider network with technical assistance and support regarding identified problems and issues that may need to be addressed on an individual or agency basis.
      • Provide support/guidance to the unit and agencies on agency special initiatives as well as related activities and resources associated with same.
      • Represent TCMU as it relates to special initiatives, as assigned
      • Establish and maintain effective working relationships with internal departmental and external system providers and officials.

      Skills Required:

      • Working knowledge of mental health and substance use disorders
      • Knowledge of multiple support and social services systems and the Behavioral Health System
      • Well versed in the Philadelphia Recovery and resilience Principles, CSP components, and Practice Guidelines *Excellent oral and written communication skills
      • Self motivating
      • Strong interpersonal skills
      • Strong organization and analystic skills, detail oriented
      • Computer skills including Microsoft Word, Excel, PowerPoint, and Outlook

      Education and Experience:

      Master’s degree in Social Work, Human Services/Social Sciences, Public Health, Healthcare, or a related field required. 3 years of experience within the Behavioral Health System in direct care services. Experience with Case Management services a plus.

      Essential Functions:

      • Ability to sit and stand for extended periods of time
      • Ability to travel independently and use public transportation as required
      • Must have a valid driver’s license and an acceptable driving record.
