What to Expect (Job Responsibilities):
- Conduct in-home assessments on patients, including medication reviews and health screenings
- Provide patient education by aligning care preferences with the best available options
- Identify diagnoses for care management and medical oversight
- Collaborate with the Regional Medical Officer and team members to develop tailored care plans
- Coordinate care with various stakeholders, including primary care physicians and specialists
What is Required (Qualifications):
- Minimum educational requirement: Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Active Nurse Practitioner or Physician Assistant license
- Active Nurse Practitioner Board Certification or Physician Assistant Certification
- NPI Number, DEA, and Valid BLS certification
- Minimum of one (1) year of prior clinical or home care experience preferred
How to Stand Out (Preferred Qualifications):
- Experience in caring for older adult (geriatric) patients
- Familiarity with electronic medical records (EMR)
- Strong problem-solving, organizational, and time management skills
- Ability to communicate positively and effectively with others
