      What to Expect (Job Responsibilities):
      - Establish and maintain effective communication with clients, family members, and referral sources to process quality referrals
      - Document all referral-related information and actions taken in the medical record accurately and timely
      - Ensure timely attention to referrals received from various platforms, including eFax and telephone
      - Review eligibility alerts and responses for Medicare clients as applicable
      - Complete follow-up calls to clients/caregivers/referral sources to confirm or collect necessary information

      What is Required (Qualifications):
      - High School Diploma or GED
      - Minimum of one year of experience in home care, hospice, or personal care services
      - Prior experience with Electronic Medical Records (EMR), HCHB preferred
      - Experience dealing with various payors in healthcare, such as Medicare, Medicaid, and commercial payers preferred

      How to Stand Out (Preferred Qualifications):
      - Experience in a similar role within the healthcare industry
      - Familiarity with ICD-10 coding and related documentation
      - Strong organizational and multitasking skills
      - Ability to work effectively in a virtual environment
      - Excellent communication and interpersonal skills
