      What to Expect (Job Responsibilities):
      - Manage and support transitional care and post-acute services for members in a designated geographical region
      - Facilitate safe transitions from nursing facilities, hospitals, and other institutional settings to community-based living
      - Collaborate with Care Coordinators and Interdisciplinary Care Teams to ensure effective care transitions
      - Plan, coordinate, monitor, and evaluate options to meet individual needs prior to community transition
      - Ensure smooth handoff to the assigned Care Coordinator for ongoing support

      What is Required (Qualifications):
      - Bachelor's degree in health & human services or related fields of study (preferred)
      - Certification as a Licensed Mental Health Professional (LPC, LCSW, LMFT), Registered Nurse (RN), or Licensed Practical Nurse (LPN)
      - Minimum of one year of experience with Medicaid or CCC Plus populations (required)
      - Minimum of two years of experience in Case Management (required)
      - Strong organizational and communication skills

      How to Stand Out (Preferred Qualifications):
      - Experience in discharge planning
      - Additional certifications in relevant areas
      - Familiarity with community resources and support systems
      - Bilingual communication skills
      - Previous experience in a remote work environment
