      What to Expect (Job Responsibilities):
      - Manage and support transitional care and post-acute services for members in a designated geographical area
      - Collaborate with Care Coordinators and interdisciplinary teams to facilitate safe transitions from institutional settings to community-based environments
      - Plan, coordinate, monitor, and evaluate options to meet individual needs prior to community transition
      - Ensure smooth handoff to the assigned Care Coordinator for continued support
      - Assist individuals in maintaining their community setting during transitions

      What is Required (Qualifications):
      - Bachelor's degree in health & human services or related fields (preferred)
      - Licensed Mental Health Professional (LMHP) such as LPC, LCSW, or LMFT, or RN or LPN certification
      - Minimum of one year of experience with Medicaid or CCC Plus populations
      - Minimum of two years of experience in Case Management
      - Strong communication and organizational skills

      How to Stand Out (Preferred Qualifications):
      - Experience in discharge planning or managing transitions in care settings
      - Familiarity with community resources and support services for individuals transitioning from institutional care
      - Additional certifications in case management or related fields
      - Proven ability to work collaboratively in a team-oriented environment
