      What to Expect (Job Responsibilities):
      - Provide support to members transitioning from inpatient settings to appropriate care levels
      - Conduct on-site visits and outreach to member providers to facilitate discharge planning
      - Develop and communicate comprehensive post-discharge transition plans
      - Monitor high-risk members and coordinate care to ensure timely transitions
      - Ensure accuracy and compliance of clinical documentation and care management activities

      What is Required (Qualifications):
      - Graduation from an accredited nursing school and three (3) years of relevant experience, or a Master's degree in Human Services with at least two (2) years of relevant experience
      - Valid, active RN license in North Carolina, or a valid clinical license (LCSW, LMFT, LCAS, LCMHC, LPA) in North Carolina
      - Demonstrated knowledge of mental health and substance abuse treatment
      - Proficiency in Microsoft Office products
      - Ability to manage multiple tasks and priorities effectively

      How to Stand Out (Preferred Qualifications):
      - Certifications such as NACCM, NADD-Specialist, Health Education Specialist, or CBIS
      - Experience in behavioral health, medical, or managed care settings
