      What to Expect (Job Responsibilities):
      - Provide support to members transitioning from inpatient settings to appropriate levels of care
      - Conduct onsite visits during members' stays in various facilities and facilitate clinical handoffs
      - Develop and communicate a comprehensive 90-day post-discharge transition plan with members and care teams
      - Monitor and coordinate care for high-risk or complex cases, escalating issues as necessary
      - Ensure accurate clinical documentation and compliance with state and Medicaid requirements

      What is Required (Qualifications):
      - Graduation from an accredited school of Nursing with three (3) years of experience in care management or related fields, or a Master's degree in Human Services with two (2) years of experience
      - Valid, active RN license or clinical license in North Carolina
      - Exceptional interpersonal and communication skills
      - Ability to independently manage multiple tasks and caseloads under pressure
      - Proficiency in Microsoft Office products

      How to Stand Out (Preferred Qualifications):
      - Certifications such as NACCM, NADD-Specialist, Health Education Specialist, and/or CBIS
      - Experience working with mental health, substance abuse, and developmental disabilities
