      What to Expect (Job Responsibilities):
      - Collaborate with hospital discharge teams to review daily patient discharges and contact patients within two business days
      - Assess patients' understanding of their healthcare needs and compliance with treatment plans
      - Perform medication reconciliation to ensure safety and accuracy
      - Schedule follow-up appointments for patients with their healthcare providers
      - Follow up with patients for up to 30 days post-discharge to ensure their needs are met

      What is Required (Qualifications):
      - Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of North Carolina
      - Current BLS/CPR certification
      - Minimum of two years of case management experience preferred
      - Minimum of two years of clinical or ambulatory nursing experience preferred

      How to Stand Out (Preferred Qualifications):
      - Experience in telephonic care management or transitional care coordination
      - Strong communication and interpersonal skills to effectively interact with patients and healthcare teams
      - Familiarity with electronic health records (EHR) systems
