What to Expect (Job Responsibilities):
- Call patients post-discharge to ensure they are doing well and have received necessary assistance
- Coordinate services and care for members with affiliated vendors, including transportation, DME, meals, and home health care
- Assist in scheduling post-discharge visits with primary care physicians and specialists
- Collaborate with patients, providers, and caregivers to ensure positive health outcomes
- Participate in interdisciplinary huddles with the TOC team to address patient needs
What is Required (Qualifications):
- An active CMA/RMA certification through a nationally accredited certification agency (CCMA, AAMA, NHA)
- 2-3 years of experience in healthcare, managed care, home health, and/or community health settings
