What to Expect (Job Responsibilities):
- Attend daily interdisciplinary team meetings to discuss inpatients and suggest discharge plans
- Communicate with primary team members to address potential barriers to discharge or transition
- Collaborate with facilities and team members to ensure safe, efficient transitions of care
- Document all inpatient care transitions and case management progress notes within the electronic health record
- Conduct concurrent and retrospective utilization reviews for inpatient and observation services
What is Required (Qualifications):
- Associate of Science in Nursing; Bachelor of Science in Nursing preferred
- Licensed by the Commonwealth of Massachusetts Board of Registration in Nursing as a Registered Nurse
- Valid Driver’s License
- Three to five years of nursing experience, with at least one year working with a frail or elder population
- Strong interpersonal, analytical, and communication skills
How to Stand Out (Preferred Qualifications):
- CCM or similar certification desired
- Recent case management or utilization management experience
- Knowledge of criteria for medical necessity determination
