Wellness Coordinator/ Health Educator - La Central Residence
Comunilife is looking for a Health Educator to provide service to individuals who have been formerly homeless with special needs. The Wellness Coordinator/ Health Educator for La Central provides health education and counseling, psychoeducation and support, referrals and linkages to community resources for clients, responsibilities include planning and coordinating special events and activities. The Coordinator will also develop and implement weekly recreational activities including field trips, outings, arts & crafts, games, music and other hobbies. The Coordinator must be capable of comprehending complex public health issues and have the ability to articulate recommended prevention strategies. We offer a generous benefits and paid time off package. The salary for this position is up to $55,000 a year.
Duties:
Responsibilities include but not limited to:
• Conduct needs assessments
• Establishing a network of community resources to facilitate wellness activities
• Recommend appropriate activities, lead and co-lead activity groups and encourage clients’ participation in community events
• Provide wellness-related counseling and monitor clients’ progress with their goals
• Work collaboratively in an inter-disciplinary team to ensure comprehensive, client-centered services are delivered and documented
Qualifications:• Bachelor’s Degree in Human Services, Education or a related area
• Two (2) years’ work experience in benefits and entitlements with extensive knowledge of community resources
• Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus
• Bilingual skills in English/Spanish a plus
