      What to Expect (Job Responsibilities):
      - Provide care management services for members post-discharge from behavioral health services
      - Collaborate with members, families, and providers to ensure access to professional services and community resources
      - Offer referrals and support to maintain member safety and well-being in the community
      - Assist clinical staff in ensuring appropriate care for members and document support accurately
      - Handle inquiries from members and providers to meet care needs

      What is Required (Qualifications):
      - High School Diploma/GED or equivalent work experience
      - Must be 18 years of age or older
      - 2+ years of customer service experience
      - Proficiency in Microsoft Word, Excel, and Outlook
      - Ability to work full-time shifts between 7:00 AM - 6:00 PM CST, Monday - Friday, with flexibility for occasional overtime

      How to Stand Out (Preferred Qualifications):
      - Experience in the behavioral health field with knowledge of diagnosis and treatment options
      - Familiarity with clinical systems ICUE and LINX
      - Previous caseload management experience
      - Background in healthcare industry and health insurance
      - Direct experience working with members to assess and address identified needs
