What to Expect (Job Responsibilities):
- Coordinate, monitor, and evaluate healthcare options and services to optimize patient health status
- Collaborate with inpatient and outpatient case management to implement discharge plans and ensure safe transitions of care
- Design individualized plans of care in collaboration with patients, healthcare providers, and families
- Utilize data to drive decisions and implement performance improvement strategies for assigned patients
- Educate the multidisciplinary team about clinical pathways, protocols, and managed care principles
What is Required (Qualifications):
- Associate’s Degree in Nursing with seven (7) years of clinical experience in a healthcare setting, or Bachelor’s Degree in Nursing with five (5) years of clinical experience
- Current Registered Nurse license issued by the state or a multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC)
- State criminal background check and Federal background check, if applicable
- Certification in Basic Life Support must be obtained within 30 days of hire date
- Excellent interpersonal communication and negotiation skills

- Prior care coordination experience
