The primary focus of this position is ongoing case management, including care coordination, care plan follow-through, resource navigation, coverage coordination, and discharge planning. The Case Manager provides backup assistance to the admissions team as needed; ownership of the admissions process and census development are not the primary responsibilities of this position.

Essential Duties:

  • Resource navigation- identify resident and family needs related to adjustment, coping, support systems, and discharge planning.
  • Identify resident and family goals, preferences, adjustment needs, support systems, and barriers to care or discharge; communicate these needs to the appropriate team members and follow up on actions.
  • Support resident acclimation and satisfaction through regular resident and family contact, timely updates, and follow-through on questions or concerns.
  • Participate in team huddles to review resident progress, changes in needs, upcoming reviews, discharge readiness, and unresolved coordination issues.
  • Serves as a liaison for residents and families in accessing community resources, financial assistance programs, transportation services, and post-discharge support services.
  • Collaborate with social services and the interdisciplinary team to assess discharge barriers and coordinate community resources, transportation, financial assistance referrals, caregiver support, and post-discharge services.
  • Primary liaison for Resident episode of care and all services assigned by the comprehensive care plan and Resident satisfaction
  • Participate in QAPI and care transition initiatives, including review of discharge barriers, resident satisfaction, rehospitalizations, and continuity of care opportunities.
  • Participate in care transition initiatives aimed at reducing rehospitalizations and improving resident outcomes.
  • Coordinate the admissions process, including referral review, resident assessments, waitlist management, and collaboration with hospitals, physicians, families, and internal departments to ensure appropriate placement and timely admissions.
  • Manage admission documentation and insurance verification, including residency agreements, advance directives, Medicare and managed care coverage reviews, pre-authorizations, and communication with payers to support reimbursement and regulatory compliance.
  • Maintain census accuracy and admission-related records while partnering with billing, MDS, and clinical teams to ensure seamless exchange of resident information and authorization requirements.
  • Support quality and operational excellence through participation in QAPI activities, monitoring key admission and census metrics, and coordinating resident admissions, transfers, and discharges across departments.
  • Assist in conducting tours (when necessary) for prospective residents, family members and/or guests.
  • Participate in staff shift change huddles, reviewing any approaching admission activity.
  • Support positive workplace environment including close collaboration of interdepartmental staff teams.
  • Ensure resident rooms are equipped with appropriate and necessary durable medical equipment and/or safety provisions prior to resident arrival in collaboration with Health Unit Coordinator and central supply.
  • Coordinate and participate in interdisciplinary care plan meetings, ensuring resident goals, discharge plans, and psychosocial needs are addressed.

Optional nurse-only add-on:

  • Conduct admission assessments and provide backup support for MDS assessment completion, clinical audits, and other nursing-related admission functions as assigned. (Licensed Nurse only).

Qualifications:

  • Holds current licensure as Licensed Practical Nurse (LPN), Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) in the state of Ohio.
  • Possesses excellent written, oral, and interpersonal skills.
  • One to two years’ experience working in a long-term care facility preferred.
