Clinical Coordinator

The Clinical Coordinator provides administrative support to clinical operations and team leaders while serving as an important point of contact for patients and families. This role provides phone support to the reception area, assists with patient and family needs, and helps ensure our teams have the information and resources needed to provide compassionate, high-quality hospice care.

What You Should Know About the Clinical Coordinator Role

• This is a full-time, Monday-Friday position, 8:00 a.m.-5:00 p.m., primarily based at our Wooster office.
• The Clinical Coordinator will be expected to work from our New Philadelphia office one day per week.
• Occasional travel to other Ohio's Hospice offices may be required based on operational and staffing needs.
• You will work collaboratively with our other two Clinical Coordinators to support clinical teams and ensure consistent, efficient operations.
• This is a key support role that helps our clinical teams provide superior care and superior service to patients and families.
• The ideal candidate will have a genuine heart for hospice and a desire to support patients, families, and coworkers during an important time in their lives.

Clinical Coordinator Qualifications

• Associate degree in office management or a related field preferred.
• Proficiency in Microsoft Word and Microsoft Excel.
• Previous experience with an Electronic Medical Record (EMR) system preferred.
• Excellent communication and interpersonal skills.
• Strong organizational skills and attention to detail.
• Ability to prioritize multiple responsibilities and work effectively in a fast-paced environment.
• Ability to drive during daytime, nighttime, and inclement weather as needed.
• Valid driver's license with a safe driving record.
• State minimum automobile insurance coverage.

Key Responsibilities

• Prepare and distribute daily care manager caseload reports to team leaders, providing an accurate picture of current caseloads and helping identify and address issues that may affect quality patient care.
• Distribute lab work to care managers on the same day it is received to ensure timely access to patient information.
• Maintain PTO, holiday, on-call, and attendance records for assigned teams so team leaders have accurate information regarding staff availability.
• Coordinate assigned meetings, including contacting participants, reserving meeting space, arranging room setup, ordering refreshments when appropriate, and recording meeting minutes.
• Receive and respond to patient and family phone calls using appropriate customer service skills and promptly communicate calls to the appropriate team member.
• Document patient and family phone calls in the EMR in a timely and accurate manner to ensure effective communication among the care team.
• Prepare and distribute Quality of Life meeting materials to team members in advance of meetings.
• Review weekly synchronization reports and communicate compliance concerns to the Team Leader for follow-up.
• Print DME reports for Quality of Life team meetings and assist Team Leaders and care managers in identifying and reporting variances.
• Review payroll for accuracy and complete assigned payroll responsibilities.
• Complete audits, reports, and other administrative projects as requested by the Director or Team Leaders.
• Assist other departments and teams as needed to support comprehensive and coordinated patient care.
• Participate in Continuous Quality Improvement (CQI) activities to support the quality and effectiveness of Ohio's Hospice services.
• Travel to other Ohio's Hospice locations as needed to provide administrative or operational support.
