      Be the Best Hospice Care Consultant You Can Be

      If you meet these qualifications, we would love to meet you:

      • Build meaningful relationships with physicians, nurses, and care facility staff through daily in-person visits and educational conversations, helping them understand when and how hospice care can best support patients and families.
      • Collaborate closely with Admissions and Marketing teams to thoughtfully develop and carry out community outreach plans that reflect the mission of honoring life and offering hope.
      • Provide consistent education and compassionate guidance to referral sources, ensuring patients and their loved ones are introduced to hospice care at the right time with clarity and care.
      • Maintain accurate and timely documentation in the CRM system while tracking outreach efforts, admissions, and community interactions to support continuity and accountability.
      • Partner with clinical staff to ensure each referral transitions smoothly into care, always prioritizing patient comfort, dignity, and family support during this sensitive time.
      • Uphold ongoing commitments to quality, ethics, and learning by participating in trainings, planning thoughtful outreach strategies, and honoring confidentiality in all aspects of your work.

      Responsibilities:

      • Bachelor’s degree preferred, or 5+ years of experience in hospice, home health, healthcare sales, or marketing in lieu of a degree.
      • Excellent written and verbal communication skills; strong ability to collaborate with a wide range of stakeholders and interdisciplinary teams.
      • Skilled in problem-solving, understanding referral patterns, and navigating or learning Medicare regulations and hospice reimbursement practices.
      • Proficient with Microsoft Office Suite; available for after-hours and weekend referrals as needed.
      • Physically capable of frequent standing, walking, typing, and working on hard/rough surfaces; occasional lifting, kneeling, reaching, and exposure to chemicals, dust, extreme temperatures, and other environmental conditions.
      • Maintains constant visual, auditory, and verbal engagement; able to drive, adapt to varying indoor/outdoor settings, and complete all documentation and HCC competencies with attention to compliance and detail.
