Job Description

- To be responsible to answering the telephone, dealing with queries and relaying relevant information to members of the team.
- To ensure brokerage updates are updated on the patient board and wards informed where necessary.
- To support the Discharge Team with general administrative duties
- To assist with the processing of patient referrals on the Local Authority System
- To be responsible for updating the Local Authority System with confirmed discharge dates and confirming actual supported discharge figures for any given day
- To assist the Hospital Social Services Team with obtaining relevant health information for patients to aid with the assessment process.
- Experience in hospital administration setting
- Excellent verbal and written communication skills
- Ability to work under pressure.
- Answering the telephone in a professional manner and directing telephone queries to the relevant team member, taking messages where applicable
- To respond and deal with email queries promptly, efficiently and politely, resolving local issues and referring to other relevant staff as appropriate maintaining strict confidentiality at all times.
- To maintain an accurate record on various spreadsheets/trackers and ensuring this information is updated as and when required
- To maintain and utilize relevant hospital and social services systems, ensuring information is accurate and up to date.
- To assist the discharge team with printing and scanning of documents when required.
- To utilize diaries, rotas, movement boards and relevant information to ensure effective communication throughout the team, including setting up meetings and taking minutes when required.
- To assist the discharge team with package of care queries and relaying this information in a timely manner
- To ensure all brokerage, placement and out of borough updates for patients on supported discharge are documented on EPIC.
- To update EPIC with relevant supported discharge information for patients as and when they are received.
- To attend hurdles at different intervals of the day and updating relevant information on both the hospital and local authority systems.
- To assist with the processing of incoming referrals on the Local Authority system for the Hospital Care Management Team
- To assist with updating the Local Authority system with hospital admission details for known patients with an open support plan.
- To assist with updating discharge information on the Local authority system for patients that have been discharged from hospital and notifying the relevant professional/team on the system

Job summary
The post holder will be responsible for providing a quality administrative service to professionals involved in the admission and safe discharge of adult patients in and out of King's College Hospital, including Orpington Hospital. To be responsible for the accuracy of patient information as it transfers from King's data systems to those of the Local Authority ensuring data accuracy and quality. To support patient flow across site and promptly deal with telephone and email queries in a professional manner.
