      What to Expect (Job Responsibilities):
      - Provide one-on-one and group coaching to Care Coordinators to enhance patient engagement and care coordination
      - Conduct regular quality reviews and offer constructive feedback to improve team performance
      - Develop and deliver training materials and job aids on workflows and soft skills
      - Collaborate with Managers and Supervisors to support team goals and performance objectives
      - Analyze team performance data to identify trends and opportunities for process improvement

      What is Required (Qualifications):
      - High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred
      - Significant experience in a Care Coordinator or similar patient-facing healthcare role
      - Proven experience in training, mentoring, or coaching peers
      - Expert-level proficiency in relevant workflows and documentation tools
      - Exceptional communication, interpersonal, and active listening skills
