What to Expect (Job Responsibilities):
- Assist the RN Case Manager in performing case management functions under direct supervision
- Review medical data to update case management plans and communicate with healthcare providers and payers
- Verify medical necessity of care or equipment ordered and make care arrangements according to patient needs
- Document case actions and maintain timely reports in the computer system
- Facilitate communication with patients and providers to ensure continuity of care and address any non-medical issues impacting patient welfare
What is Required (Qualifications):
- Current active and unrestricted License as a Practical Nurse or Medical Assistant/Nursing Assistant Certification in the U.S.
- Minimum of 5 years of varied clinical experience preferred, or 3 years in a specialty field
- Basic computer skills for documentation and communication
- Ability to operate standard office equipment and maintain accurate records
- Strong communication skills to liaise with patients, providers, and payers
How to Stand Out (Preferred Qualifications):
- Experience in case management or familiarity with healthcare reimbursement processes
- Additional certifications in healthcare or case management
- Knowledge of community resources and funding sources for patient assistance
- Proficient in using case management software and tools
- Experience working with chronic condition management
