      Buckinghamshire Healthcare NHS Trust are currently seeking organised and motivated candidates to succeed in the position of a Band 6 Case Manager under a long-term placement contract.

      Main duties of the job

      The Case Manager roles main focus is to coordinates patients discharge using multidisciplinary integrated care plans, which is able to support their on-going care. This role will include agreeing on the appropriate goals are set for each patient, which can be reviewed and working in partnership with Care Hubs and Home Care agencies to aid in their reablement and discharge. There will be a requirement to assist patients through their care hub pathway, ensuring the suitable interventions and rehabilitation are flexibly taking place to minimise any need for long-term care.

      This position is fully remote so the best candidate for the role would have to have a proficiency with Microsoft Office and a high level of IT skills. They would also have to be excellent communicators, able to have correspond with different departments and groups in a multi-disciplinary team, as well as being able to manager difficult conversations to ensure the best pathway is taken for candidates. Having clinical experience and experience in discharge planning is required in order to have the proper skillset to coordinate the most efficient discharge.

      Job description

      Job responsibilities

      The single point of access receives referrals from primary care, secondary care, community health and social care professionals, ambulance service, NHS111, VCS, and care homes, for patients who need clinical interventions from community health services for admission avoidance and on discharge home from hospital. This includes pathways which are an alternative to the emergency department, to enable patients to be seen at home or in the right place, at the right time. The single point of access is called the Community Care and Coordination Team (CCCT)

      This post is for a registered clinician to work within the single point of access to undertake robust clinical triage and be a senior clinical decision maker, identifying the most appropriate clinical pathway for the patient to enable them to be seen in the right place, by the right professional and not be unnecessarily conveyed or admitted to hospital.

      The post holder will work in close partnership with the acute sector, social and community service colleagues. They will have in-depth clinical knowledge and experience, with high understanding of services and pathways to enable appropriate decision-making and problem solving with referrers to address patients clinical needs. This includes the management of clinical risk, coordination of referrals and be accountable for the appropriate utilisation of clinical capacity across the system.

      The post holder will liaise at senior level with relevant medical, nursing and therapy staffing to ensure the patient is safely accepted onto the most appropriate clinical pathway, responded to by the most appropriate clinician and receives timely assessment, interventions and diagnostics as required. They may need to liaise with social services, voluntary sector, patients and their families as appropriate.

      The appropriate pathways as alternative to ED include, but not limited to:

      SDEC and FSDEC

      Hospital at Home pathways

      Urgent community response and Rapid Response and Intermediate Care

      MUDAS and CATS

      Adult Community Healthcare Teams
