      Responsibilities

      1. Services to individuals

      • Treat all clients in a respectful and dignifying manner.
      • Engage clients in a working relationship that is appropriate to their service needs.
      • Assist clients with the self-administration of medication as necessary, including:

      1. Following agency policies and procedures,

      2. Proactively identifying and resolving or reporting problems,

      3. Helping people move towards greater independence managing their medication.

      2. Help clients access and utilize the services and supports they want and need, by:

      • Providing referrals,
      • Helping clients to establish or improve relations with their natural supports and their service providers,
      • Advocating as necessary to ensure the receipt of optimal services.

      3. Anticipate, prevent, and respond to crises, by:

      • Working with the client to identify and resolve factors that may lead to a crisis,
      • Recognizing as early as possible when a client is in crisis or moving into one, and intervening as rapidly as possible to address the situation, including working to join with the client in identifying and addressing it and engaging other supports as necessary,
      • Using as person centered an approach as possible,
      • Informing supervisory staff and requesting necessary assistance as early as practical.

      4. Services to Groups or Classes

      • Adequately prepare for each meeting of the group or class, and begin and end meetings on time,
      • Ensure coverage when unable to lead the group or class, and provide adequate notification to participants if coverage is unavailable,
      • Ensure that purpose of the group or class is clear and well understood by the participants,
      • Engage participants in working together to accomplish the purpose of their gathering,
      • Follow prescribed guidelines and curricula when available,
      • Keep other staff appropriately informed about the participants’ involvement in the group or class.

      5. Engagement With Collateral and Community Resources

      • Engage collateral providers and family members as appropriate,
      • Interact with external parties in a manner that is courteous, and that supports the development of an effective working relationships,
      • Adhere to agency policies regarding confidential information, and otherwise respect and protect the privacy of service recipients.

      6. Service Documentation

      • Complete progress notes according to agency and funding source guidelines regarding timeliness,
      • Complete all other service documentation according to agency and funding source guidelines regarding timeliness,
      • Achieve on time completion of service documentation without ongoing reminders from supervisor, independently use Anasazi reports as necessary to do this,
      • Provide documentation that is accurate, relevant, concise, appropriate to the setting, and supportive of quality services.

      Experience and Education Required:

      • C M 1: High school diploma and 2 years experience
      • CM 2: Bachelors degree or HS diploma or equivalent and 4 years of relevant work experience
      • CM 3: BSW and 1 year relevant experience (excluding fieldwork) or Bachelors degree and 2 years relevant experience or HS diploma and 6 years relevant experience
      • CM 4: 3 years as CM 3 (internal only)
      • CM 5: 3 years as CM 4 (internal only)
      • For applicants without college degrees, every 30 credits can be substituted for 1 year of experience
      • Demonstrated ability to provide services to a specialized population, computer literacy and good written and verbal communication skills are required.
      • Must be able to work effectively as part of a multidisciplinary team
