      Case Manager

      Provides comprehensive case management and supportive services to clients and families, including assessment, advocacy, progress monitoring, documentation, and collaboration with community resources to promote self-sufficiency and well-being.

      Job Responsibilities

      1. Provide case management and supportive services to clients and/or their families on order to promote self-sufficiency.

      2. Responsible for developing, monitoring, and evaluating client's progress

      3. Meets with each client as required by the program or based on client's needs.

      4. Advocates for clients with regards to financial, state/federal benefits, housing, education, childcare, skill testing and development programs, job training, job referral and/or treatment programs

      5. Completes psycho-social and other assessments in client's home, Agency properties, or other locations accessible to the client, including but not limited to: individual interviews, collateral conferences, evaluation of client needs and abilities and following-up, when necessary.

      6. Maintain up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies while performing job expectations; Responsible for maintaining routine and ad-hoc reports.

      7. Develops and facilitates scheduled monthly support group meetings, establishes services plans and intervenes, when necessary, using a variety of professional approaches.

      8. Knows and complies with laws and regulations relevant to the position and services.

      9. Participate in community outreach efforts

      10. Collaborates with other professionals in assisting the client in meeting the goals of the service plan. Follow established procedures for any concerns related to security or safety on campus.

      PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.

      X Kneel and move from sitting, bending, kneeling or standing multiple times a day.
      X Push and pull objects up to 25 pounds.
      X Climb up and down up to 3 flights of stairs at a time.
      X Lift up to 25 pounds.

      X Agency-specified automobile insurance
      X Additional Requirements: May be required to occasionally work weekends, or special events.

      Education and Experience Requirements:

      Relevant Education:
      Preferred: Master's degree in social services or relevant field
      Minimum: Bachelor's degree in social services or relevant field


      Relevant Experience:
      Preferred: 5 years of case management experience, LCPC, or LCSW

      Minimum: 3 years of case management experience, LCPC, or LCSW

      Certification/Licensure:
      Preferred: N/A
      Minimum: 40-hour Domestic Violence Certificate within 6 months of hire
