Job Description

- Conduct comprehensive assessments to identify veterans’ needs, strengths, and barriers to employment.
- Develop and implement individualized service plans, coordinating employment and supportive services.
- Provide case management support, including referrals for housing, mental health services, substance use treatment, transportation, and other essential resources.
- Work closely with the Employment Specialist to align job placement efforts with participants’ skills and career goals.
- Monitor and document participant progress, ensuring compliance with HVRP grant requirements.
- Advocate for veterans and assist in navigating systems such as VA benefits, workforce development programs, and community services.
- Facilitate life skills workshops and employment readiness training.
- Maintain accurate case notes and data tracking for program reporting and performance evaluation.
- Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
- Perform all other duties as assigned.
- Knowledge of community resources, veteran benefits, and employment support programs.
- Strong problem-solving and crisis intervention skills.
- Experience working with homeless populations, individuals with substance use disorders, or mental health challenges is a plus.
- Proficiency in case documentation and program reporting.
- Commitment to supporting veterans in overcoming challenges and achieving self-sufficiency.
- Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.
- Professional appearance and demeanor.
- Military/Veteran preferred.
- Must have the ability to obtain and maintain a successful background check.
- Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required.
- CPR Certification, First Aid Certification, and an annual Tuberculosis test.
- During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
- Minimum 2 years of experience in case management, workforce development, or veteran services.
- Graduation from an accredited four-year college with a degree in Social Work, Human Services, Business Administration, or related field is preferred.
- High School diploma or its equivalent is required.

POSITION SUMMARY:
