      Position Purpose

      Under the supervision of the Case Management (CM) Supervisor, the Case Manager completes case management activities for Rocky Mountain Human Services (RMHS) clients. Case management activities will include but will not be limited to evaluating applications and documentation to determine eligibility for developmental disability services in accordance with state and federal guidelines, appropriate follow up activities, and completing all documentation in the expected timeframe. The Case Manager I will apply appropriate criteria, guidelines, and regulations specific to the level of care and services required to meet the member/family goals and the organizational/contractual requirements. Your work ensures that individuals with developmental disabilities receive access to critical services and supports that promote independence, inclusion, and quality of life.

      Essential Duties

      · Conduct thorough reviews of applications, medical records, psychological evaluations, educational reports, and other relevant documents.

      · Evaluate applicant eligibility for Developmental Disability services using established criteria and assessment tools.

      · Communicate with applicants, families, guardians, medical professionals, educators, and other stakeholders to gather supplemental information as needed.

      · Write comprehensive determination reports that clearly document findings and support conclusions.

      · Maintain accurate and up-to-date records in compliance with state regulations and agency policies.

      · Provide clear and compassionate explanations of eligibility decisions to applicants and families, including guidance on appeal processes when applicable.

      · Stay current on changes to DD eligibility criteria, assessment tools, and best practices through ongoing training and professional development.

      · Uphold strict confidentiality and ethical standards in all aspects of case management.

      · Establish professional and effective collaboration, communication, and coordination among all responsible parties of an individual member’s interdisciplinary health care team.

      · Maintain knowledge of regulations, policies, and procedures regarding current public assistance programs.

      · Assist clients and providers in understanding the complaint, grievance and appeal process.

      · Responsible for accurate and timely completion of all forms, reports, and documentation of care management activities.

      · Maintain professional and ethical manner with all interactions and meet performance, quality, customer service, and coordination standards as assigned by the department management team.

      · Participate in training and staff development opportunities. Actively participate in team meetings and communicate progress and barriers with Supervisor and/or Program Manager or Department Director.

      Qualifications


      Knowledge, Skills and Abilities

      · Ability to process high volume of work efficiently with a high level of customer service detail.

      · Knowledge of and ability to relate to populations served by the programs for which we work, client interviewing and assessment skills, knowledge of policies and procedures regarding public assistance programs, ability to develop care plans and service agreements, knowledge of resources, and negotiation, intervention, and interpersonal communication skills.

      · Demonstrates support for the company’s mission, vision and values.

      · Excellent written and verbal communication skills, strong organizational and time management skills, strong interpersonal skills and the ability to handle multiple priorities.

      · Ability to work independently outside of office setting and conduct face to face assessment visits. Must have general computer skills and ability to work with Microsoft Office products.

      · Ability to use the complaint, grievance and appeals process and procedures to facilitate quality outcomes and/or resolutions for clients.

      · May be required to manage multiple priorities and projects with tight deadlines.

      Minimum Qualifications

      · Bachelor’s degree in human behavioral sciences field.

      Preferred Qualifications

      · Prior care management experience preferred.
