      Job Description

      We are seeking a dedicated and compassionate Case Manager to join our team. In this role, you will be responsible for communication and relationship building with clients and all associated partners. You will set limits for youth and set goals towards individual successes in conjunction with clients’ guardians, therapists, and other providers. Participation in on-call services as needed is also required.

      Responsibilities

      • Build and maintain relationships with clients and collateral partners (schools, funding partners, therapists, agencies, foster homes, etc.).
      • Set limits for youth and develop interventions.
      • Collaborate with clients' guardians, therapists, and other providers to set individual success goals.
      • Travel as needed on a day-to-day basis to meet with client's providers.
      • Participate in on-call services as needed.
      • Other duties as assigned.

      Qualifications

      • Bachelor’s Degree in Social Work, or any Social Science.
      • LSW or LSWA license or license-eligible preferred but not required.
      • Must hold a valid Driver's License and be willing to commute as needed.

      Pay and Benefits

      The pay range for this position is $52000.00 - $52000.00/yr.

      This is a fully onsite position in Malden,MA.
