Job Description

- Complete screening with individuals who contact Catholic Charities who are eligible to receive screening through the 1115 Waiver. This will include ensuring general demographic information is completed as well as the permission of the member is acquired. Screening must be completed using the required 1115 Waiver screening tool. Document screening accurately in the Unite Us platform.
- Complete basic navigation services with individuals who contact Catholic Charities who have been screened and are eligible to receive basic navigation services through the 1115 Waiver. This will include eligibility assessment and referral management. Document basic navigation services accurately in the Unite Us platform.
- Provide housing services for those households that are eligible for these services under the 1115 Waiver. Document all housing services accurately in the Unite Us platform. Housing services will include providing the following services:
- Pre-tenancy services, including, but not limited to: assisting with rental applications, negotiating lease agreements, assisting with the set-up of a new housing unit, connection to resources aiding with housing costs and other expenses, review of living environment to ensure it meets the needs of the individual, etc.
- Tenancy sustaining services, including but not limited to: assistance in linking to free or affordable legal services, connection to resources to establish a bank account, assistance in addressing circumstances and/or behaviors that may jeopardize housing, assistance in completing applications for housing support and other social service support, assistance with housing recertification process, etc.
- Housing transition and navigation services, including but not limited to: assistance with housing search and application process, contacting perspective housing options, connection to rental assistance and assistance with applications, etc.
- Participates as a team member to provide 1115 Waiver services to eligible community members.
- Completes all required training as outlined in the 1115 Waiver including, but not limited to: Unite Us training, trauma informed care, culturally and linguistically appropriate services, compliance, and HIPPA
- Associate's degree in Human Services or a related field
- 1 - 3 years of experience working with diverse populations
