Job Description
- Req#: 529255
- Carries caseload of up to 20 residents, interviewing each resident a minimum of once a week.
- With input from residents, develops a housing plan to include goals for income, health, and housing search. Monitors progress and intervenes as necessary by setting limits, giving support or otherwise assisting resident to achieve goals.
- Maintains accurate records in case file and HMIS, including contacts with resident, family members and social agencies, etc.
- Submits End of Month and other appropriate reports.
- Coordinates with other program staff as needed regarding resident interaction within the program.
- Assists resident with navigating systems in the community, including attending appointments as needed and providing hands on assistance to overcoming barriers.
- Coordinates and advocates with referral agencies to facilitate appropriate services.
- Mediates and facilitates problem solving with residents, including conducting group meetings or individual coaching.
- Other duties as assigned by Director of Ozanam Manor
- Participate in and serve as a model for SVdP’s Culture of Philanthropy, and ensure SVdP volunteers feel welcomed and appreciated *
- Good communication skills.
- Professional appearance: Business casual.
- Ability to speak Spanish preferred.
- Knowledge of psychiatric, medical and human service delivery systems.

- BA/BS in related field preferred
- Proficient in standard computer systems (Microsoft Office)
- Ability to stand, walk and lift 25 pounds.

Job Summary:
Case Managers assist transitional housing residents in accessing care and services, advocating for resident needs, and assist in removing barriers to accessing permanent housing for residents. The case manager works with assigned residents to develop housing plans and assist residents toward income and housing goals.
The ideal candidate enjoys working as a part of a team of staff and volunteers and can think and act quickly when there is a need to pivot or address an emergency. This candidate can think critically and help guests determine a course of action that will move them toward stability. This individual can work independently while ensuring complete and thorough communication with team members. The best candidate for this post is very organized, detail oriented, comfortable with creating lines of communication with community programs, and able to encourage and support follow through from residents. The representative mist be energetic, resourceful, and reliable with strong interpersonal skills and a passion for working with vulnerable populations. This person will exhibit the demonstrated ability and desire to maintain and promote an organized, safe, and efficient work environment. The goal of this position is to provide program residents with the tools and support required to permanently end their homelessness.
