- Educational Support: Complete education assessments, create Education Success Plans, and maintain communication with schools to accommodate academic needs.
- Client Monitoring: Schedule check-ins, track attendance, and manage return-to-school documentation.
- Clinical Coordination: Conduct case consults upon admission/discharge, attend clinical staff meetings, and provide conflict resolution and case management as needed.
- Therapeutic Facilitation: Facilitate IOP and PHP sessions, manage treatment schedules, and assist with pre-treatment processes.
- Collaboration: Work with clinical teams, parents, and students to implement treatment plans and discharge planning.
- Parent Communication: Update parents on health issues, progress, and other concerns through phone contact.
- Administrative Duties: Document progress in reports, teach life skills, and attend treatment team meetings.
- Experience working in education and behavioral health a preference
- Proficiency in assisting the administration of suicide screens such as C-SSRS
- Effective communication skills
- Ability to organize, prioritize and manage schedules and deadlines
- Ability to remain in control during stressful situations and facilitate conflict resolution
- Ability to be both firm and friendly and demonstrate appropriate boundaries with students
- Maintain a professional demeanor at all times

- First Aid & CPR certified
- B.A. in Social Services field or combination of education and experience equal to a minimum of four years relative experience and training in youth services or mental health treatment. Master’s Degree in related field of study strongly preferred.

Overview
Salary Range : $55,000 - $65,000 a year
Location: 1445 Research Blvd Suite 100, Rockville, MD
Schedule: Monday - Friday (Full-time)

As an Embark Case Manager you will be supported by an experienced Clinical Director and Administrative Team that are all focused on the mission to facilitate lasting positive change for young people and their families at the IOP and PHP levels of care. Come grow with us!
Case managers focus on coordinating and advocating for services and work closely with therapists and mental health professionals and ensure clients receive the appropriate therapeutic interventions. Collaborate with the treatment team to develop treatment plans, monitor progress, and make referrals to therapy services when needed. Play a crucial role in facilitating access to therapy services by coordinating appointments, assisting with insurance, financial matters, and helping clients.
Embark operates facilities across the US that support adolescents, young adults and their family’s mental health and well-being by offering a full continuum of care from Long Term Residential to our IOP/PHP clinics to virtual outpatient care. Our local IOP & PHP clinics are a critical part of our strategy to increase access to life-saving mental health treatments for families with insurance.
