      Sandstone of Tucson is a 240-bed skilled nursing facility committed to providing compassionate, high-quality care. We are looking for a dedicated Case Manager to join our care coordination team and help ensure each resident receives personalized, timely , and efficient support throughout their stay.

      What You’ll Do:

      • Coordinate all aspects of resident care from admission to discharge

      • Communicate regularly with residents, families, providers, and insurance case managers

      • Participate in care plan meetings and ensure proper documentation and updates

      • Facilitate discharge planning, home health referrals, DME orders, and follow-up care

      • Ensure all services are aligned with payer guidelines and maintain up-to-date authorizations

      • Track and document resident progress, status changes, and care transitions

      • Support compliance with state, federal, and facility standards

      What You’ll Bring:

      • 2+ years of experience in case management, discharge planning, or care coordination

      • Experience in a skilled nursing facility, hospital, or healthcare setting preferred

      • Strong knowledge of managed care processes, Medicare, Medicaid, and insurance plans

      • Ability to work collaboratively across departments and with external agencies

      • Excellent communication, critical thinking, and organizational skills

      Education & Requirements:

      • Bachelor's degree in Social Work , Nursing, or related field (preferred)

      • Arizona Level One Fingerprint Clearance Card

      • Covid-19 vaccination or approved exemption

      • Bilingual (Spanish/English) preferred
