Job Description
- Req#: 95191
- Coordinates resources and provides information tailored to client needs.
- Efficiently documents all resource coordination activities.
- Coordinates referrals for client-specific discharge planning, ensuring active family participation in creating a comprehensive discharge plan that reflects their choice of service providers and community resources.
- Monitors record-keeping, compliance, and remaining authorized units, ensuring client charts are current and include necessary documentation, such as incident reports within 24 hours when required.
- Assesses, monitors, and documents safety concerns.
- Completes case documentation in compliance with standards set by Merakey, county MHDS/CASSP, OMHSAS, and other relevant licensing, regulatory, and payor organizations.

- Bachelor’s Degree required, preference is in psychology, sociology, social work.
- 1-3 years of experience working with children.
- Valid Driver's license and working vehicle
