Job Description
- Req#: 79AEAD9056
- Review Community Health Worker (CHW) appointment documentation for accuracy and completeness, escalating issues to the nurse case manager as needed.
- Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers.
- Mail and/or email patient care plans and educational materials as directed by case managers.
- Accurately update and maintain patient records within the electronic health record (EHR) system.
- Protect patient confidentiality and support compliance with HIPAA and organizational policies.
- Certified or Registered Medical Assistant (CMA/RMA) required.
- 2 years of experience in a medical office, telehealth, or care coordination setting.
- Experience using EHR systems; experience using eClinicalWorks preferred.
- Experience working within HIPAA and organizational policy requirements in a healthcare setting.
- Available during Eastern Time business hours.
- Experience working with older adult populations preferred.
