      What to Expect (Job Responsibilities):
      - Provide ongoing proactive services through monthly calls to patients
      - Review patient health status and escalate concerns to physicians as necessary
      - Educate patients on medical conditions and factors affecting their health
      - Set health-related goals based on patients' medical conditions and priorities
      - Maintain and update care plans using proprietary software while ensuring productivity metrics are met

      What is Required (Qualifications):
      - Certified or Registered Medical Assistant required
      - Minimum of 2 years of experience in healthcare or a related field (3-5 years preferred)
      - Experience in family practice, internal medicine, or geriatrics preferred
      - Ability to work full-time (40 hours per week)
      - Strong communication and interpersonal skills (written and verbal)

      How to Stand Out (Preferred Qualifications):
      - Experience working in a remote healthcare setting
      - Knowledge of Electronic Health Records and proficiency with Microsoft programs and Google Suite
      - Demonstrated ability to engage effectively with colleagues and healthcare professionals
