What to Expect (Job Responsibilities):
- Follow individualized care plans to ensure safe and competent care for clients
- Assist clients with personal hygiene, mobility, and meal preparation
- Perform light housekeeping duties to maintain a clean environment
- Monitor and report changes in clients' health or home environment to the RN Case Manager
- Provide essential healthcare services in clients' homes
What is Required (Qualifications):
- Active Certified Nurse Aide (CNA) certification in good standing, listed on the applicable state nurse aide registry
- Current CPR First-Aid certification
- Reliable transportation for patient visits, if applicable
How to Stand Out (Preferred Qualifications):
- Prior experience in home health, hospice, or skilled nursing facility
