      What to Expect (Job Responsibilities):
      - Outreach to oncology customers for care coordination and management
      - Conduct clinical assessments, health education, and utilization management
      - Develop and monitor individualized plans of care
      - Document all customer encounters and provider interactions
      - Evaluate each case for quality of care and report issues

      What is Required (Qualifications):
      - Registered Nurse with 1-2 years of Case Management experience
      - 1-2 years of oncology nursing experience
      - Medicare and/or Medicaid experience with geriatric populations
      - Certified Case Manager preferred
      - Experience with community resource organizations

      How to Stand Out (Preferred Qualifications):
      - Experience managing customers with complex medical and behavioral health concerns
      - Spanish language ability is desirable
