      The Community Living Center (CLC) Nurse Manager of Programs functions as a clinical expert and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care. The NM assesses, plans, implements, and evaluates delivery of care based on age specific components, and assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction by overseeing the CLC program.

      Duties


      The CLC Nurse Manager of Programs has direct oversight for staff in the following programs: Minimum Data Set (MDS) Coordinators, Resident Assessment Coordinator (RAC), Wound Care, Restorative Care Coordinator, Falls & Safe Patient Handling and Mobility Coordinator, Admissions & Discharge Coordinator, and Clinical Nurse Leader. The CLC Nurse Manager of Programs works collaboratively with other members of the CLC Leadership team to ensure quality care is provided to our Veterans. The CLC NM directly supervises assigned nursing personnel in the Community Living Center, a 120-bed service line providing long term care, skilled care, rehabilitation, and hospice services. The NM is accountable for adhering to established policies, standards of care and standards of practice.
