      Required Qualifications:

      • Upon hire: Oregon Registered Nurse License
      • Upon hire: In compliance with the Providence Health and Services-OR Human Resources policy #302 Background Checks, a criminal background check is required for this position, based on assigned duties.

      Preferred Qualifications:

      • Bachelor's Degree Nursing.
      • 1 year Full-time experience in home health or hospice.
      • 1 year Full-time experience in home infusion.
      • 1 year Full-time experience in discharge planning or care management.
      • Experience in home care and in positions that required extensive customer service skills.
