What to Expect (Job Responsibilities):
- Serve as the single point of contact for member assessment and service coordination
- Facilitate the person-centered planning process and implement person-centered thinking
- Consult with members and families regarding behavioral and physical health care needs
- Monitor service delivery and evaluate the Person-Centered Service Plan on an ongoing basis
- Maintain accurate documentation of contacts, treatment plans, and assessments in electronic records
What is Required (Qualifications):
- High School Diploma or GED equivalent required
- Minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients
- Must have a valid driver’s license, vehicle, and verifiable insurance
- Ability to communicate effectively with a diverse group of individuals
- Intermediate proficiency with Microsoft Office, including Outlook, Word, and Excel

- Experience in discharge planning, case management, or home/community health management
- Knowledge of local, state, and federal healthcare laws and regulations
- Strong understanding of cultural and demographic diversity
