      Posting Salary Range: $35,280 - $40,950

      Office Location: Remote in Catchment.

      What We’re Looking For

      Trillium Health Resources is seeking a Community SOC Family Partner to join our System of Care (SOC) team. This position will be filled by someone who has a minimum of four (4) years of experience as a primary caregiver for a child or youth receiving Medicaid or State funded BH services. The knowledge gained from lived experience is beneficial in this role. The Community SOC Family Partner works directly in the communities that Trillium serves and is responsible for educating parents/caregivers about how to effectively navigate the child-serving physical, behavioral and social service systems for themselves and about the existence of informal/community resources available to them. This position ensures members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health that helps to establish positive outcomes for members and transform their lives for the better. While this position entails the option of a remote office location, the position does require field-based support and travel to meet the member and family needs.

      On a typical day, you might:

      • Work with member’s families and other members of the care team to identify, document and attempt to address barriers to relocation to a more integrated setting.
      • Provide linkage to needed physical psychological, behavioral and educational evaluations and supports, as appropriate for the individual’s needs.
      • Educate and train members and the member’s family or guardians on resource availability, and how to independently access resources to maintain self-sufficiency in caring for the member in the community.
      • Support transitions of care e.g. members discharging from residential or inpatient settings, transitioning out of school-related services, members experience a change in employment or change in primary care giver.

      Qualifications:

      Education: High School Graduate or GED.

      Experience: Minimum of four (4) years of experience as a primary caregiver (a family member, friend, or neighbor who provides paid or unpaid assistance to a person with a chronic illness or disability condition) a child or youth receiving Medicaid or State funded BH services.

      Preferred two (2) years paid experience working directly with individuals with BH or IDD conditions. Paid experience related to the counties in which they are applying.*this experience should be clearly documented in your resume

      License/Certification: Certified Peer Support Specialist preferred.

      Must have a valid driver’s license

      Must reside in North Carolina to be considered for remote status.

      This position requires you to work in Martin, Tyrell and Washington Counties of Trillium's catchment area.
