      Job summary

      To provide health improvement expertise and support people with Type 2 Diabetes through delivery of healthy living interventions. Health Coach will be helping people gain and use the knowledge, skills and confidence to become active participants in their care so that they can reach their self-identified health and wellbeing goals.

      Main duties of the job

      Supporting patients with 1-1 interventions for health coaching to support self-management of type 2 diabetes with the use of coaching models and motivational interviewing

      Attend monthly cooking classes with patients

      Support on weekly walking groups

      Working alongside primary, specialist care, physical, MH services and social care services

      Use of Motivation Interviewing and other behaviour changes strategies to increase patients' skills and confidence to manage their own condition and set achievable goals, and engaging patients in making informed decisions.

      Weight management, physical activity, smoking cessation and alcohol awareness support

      Establish and maintain strong and effective partnerships with teams within partner organisations who can influence and deliver the Health Improvement and Inequalities work streams e.g. Health care staff, Local Authority staff and Voluntary sector

      To provide Health Education to people to improve health and encourage healthy lifestyles and promote behavioural change.

      • Health Trainer, Health Coach qualification or equivalent knowledge
      • Background in Health / Social Care or community development
      • Training in Diabetes education or management
      • At least two years' experience of working in health improvement, public health or a relevant health improvement area
      • Full driving license

      Experience

      • Experience of planning, developing and facilitating sessions to service users
      • Experience of working with groups and facilitating group development
      • Able to be trained and then deliver appropriate training (train the trainer module).
      • Partnership working, negotiation and problem-solving skills.
      • Experience of working with local community groups and health professionals.
      • Experience of working in a multi-disciplinary team.
      • Experience of planning and organising events and delivering against projects.
      • Experience of providing one to one and group intervention for healthy lifestyle
      • Experience delivering NHS Health Check or similar physical health checks
      • Experience of delivering smoking cessation, weight management, healthy eating and alcohol awareness support
