What to Expect (Job Responsibilities):
- Facilitate completion of patient health evaluations (CHE) in nephrology offices, clinics, or via telehealth
- Perform outbound calls to patients and clinics for scheduling and reminders
- Welcome new patients and collect information on their concerns for appropriate escalation
- Maintain accurate documentation and communicate updates to the care team
- Organize outreach efforts effectively to maximize patient engagement
What is Required (Qualifications):
- High School Diploma or GED required; Associate's or Bachelor's Degree preferred
- 2+ years of professional work experience in customer service, patient-facing, or telephonic outreach roles
- Prior experience in a healthcare setting preferred
- Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook
- Strong verbal and written communication skills, along with excellent time management abilities
