      What to Expect (Job Responsibilities):
      - Confirm patient eligibility and primary care provider (PCP) assignments daily for scheduled visits
      - Assist members with the recertification process to maintain Medicaid and Medicare eligibility
      - Maintain knowledge of state and federal eligibility requirements for Medicaid and Medicare beneficiaries
      - Coordinate with member relations and care teams for timely discharges and transitions out of the organization
      - Serve as the subject matter expert on assigned state eligibility and payer eligibility websites

      What is Required (Qualifications):
      - High School diploma required; Bachelor's degree preferred
      - 1+ years of experience with Medicaid, Medicare, and/or health plan coverage and eligibility requirements
      - 1+ years of experience serving complex populations in a healthcare setting
      - Proficient computer skills, including Microsoft Office Suite and electronic medical record documentation
      - Good written and oral communication skills for effective collaboration with co-workers and state agencies

      - Experience working directly with members in a healthcare environment
      - Knowledge of community resources to assist members with healthcare barriers
      - Familiarity with state eligibility and payer eligibility websites
      - Strong organizational skills for managing multiple tasks and responsibilities
