What to Expect (Job Responsibilities):
- Deliver home-based medical care to patients within an assigned geographic area as part of an interdisciplinary team
- Collaborate with primary care providers and other health professionals to develop and implement patient care plans
- Conduct comprehensive assessments for newly enrolled patients and provide ongoing care as needed
- Educate patients and their representatives about disease management and promote wellness
- Participate in quality initiatives and ongoing meetings to enhance patient care and clinical excellence
What is Required (Qualifications):
- Certified Nurse Practitioner or Physician Assistant through a national board
- For NPs: Graduate of an accredited master's degree in nursing program with board certification
- For PAs: Graduate of an accredited Physician Assistant master's degree program with board certification
- Active and unrestricted license in the State of Ohio, with DEA licensure or prescriptive authority
- Proven knowledge of long-term care and community resources related to patient care delivery systems
How to Stand Out (Preferred Qualifications):
- 1+ years of clinical post-graduate experience in Primary Care, Home-Based Medical Care, Geriatrics, or Emergency/Urgent Care
- Understanding of Geriatrics, Chronic Illness, and Advanced Illness care
- Familiarity with CPT, ICD-10, and HCPC coding
- Proficient computer skills for documenting medical information
- Ability to develop and maintain positive customer relationships
